You must shower thoroughly with Body Wash & Shampoo before every float

Do not float if:

• You suffer from incontinence, diarrhoea, infectious diseases or uncontrolled epilepsy
• You are menstruating or breastfeeding
• You have a special or lacquered hairdo, or have applied hair colour or used tanning products within the last week
• You have open or bleeding wounds
• You have been medically advised not to float
• Please do not add perfumes, oils, bath salts or other chemicals to the water, do not smoke, drink or eat in the tank

Check with your health practitioner before using a floatation tank if you are pregnant or are concerned about heart conditions, asthma, sensitivity to sulphate or magnesium, severe skin conditions such as psoriasis and eczema, or psychosis.

I understand that I need to claim full responsibility for my health, well-being and safety in relation to my floatation experience at Infinity Float Centre. My responsibilities include, and are not limited to the following:

1. Poor standards of hygiene will not be tolerated. I accept responsibility for any costs incurred for any need to repair damage or re-establish Infinity Float’s high standards of hygiene due to my misuse of Infinity Float property and facilities.
2. I hereby agree to pay a cleaning fee of $1000 on the day of incident, should I voluntarily or involuntarily defecate, urinate or discharge any other bodily fluid in the float tank. (Please note that women on their menstrual cycle must reschedule. There will be no penalty fee)
3. I agree that I have not received any medical advice from Infinity Float and that the obtaining of any medical advice I may require or need is my own responsibility.
4. I accept that it is my responsibility to obtain advice from my health care practitioner or doctor in relation to floating and my existing health condition(s), including pregnancy, and to act accordingly.
5. I agree that I will not hold Infinity Float or any of their staff in any way responsible for any aggravation of any existing medical conditions that may occur from floating or any onset of any new health conditions which may coincidentally occur around, or soon after, the time of floating.
6. I hereby confirm and understand that Flotation Therapy can cause intense relaxation and thereby, could influence motor skills and my ability to drive or operate heavy machinery. I hereby take all responsibility for my actions during my experience and upon exiting Infinity Float.
7. I do not have a condition or am medicated in any manner which may be adversely affected by profound relaxation and/or immersion in concentrated magnesium sulfate (Epsom salt) water solution
8. I hereby confirm that I am not wearing a pacemaker, and do not have any serious heart disease or alternatively, I have consulted my Doctor and understand all associated risks of Flotation Therapy in combination with my specific medical conditions.
9. I hereby confirm that I have no history of ear infections or alternatively, I understand all risks associated with Flotation Therapy and my condition.
10. I hereby confirm that I am using the flotation facility at my own risk and I will not hold Infinity Float or any of their staff in any way responsible for any injury. I understand that while using the flotation facility I could fall due to slippery surfaces resulting in severe injury.
11. I hereby acknowledge that staff are not trained in transfer, and I must have my own helper if assistance is required getting in and out of the float pools.
12. I hereby confirm that I do not suffer from epilepsy, psychosis, respiratory, kidney or communicable diseases. In rare cases, Flotation can cause nausea, vomiting, dizziness, and rashes. These could be a sign of a Kidney disorder and an inability to process magnesium. Should these symptoms occur, I will stop using the Floatation therapy immediately and consult my Doctor.
13. I do not have any communicable or infectious disease, illness, or skin disorder. I do not currently have any open cuts, scrapes or rashes.
14. I feel well today & do not have incontinence, diarrhea or nausea
15. I understand that floating is a unique and varied experience and I may not enjoy it. No guarantee has been made that it will be beneficial for me. I further understand that there are no refunds and I take full responsibility for damages to myself or to the facility.
16. I am over the age of 16.
17. I have not had my hair colored or chemically treated within the last week.
18. I agree to shower thoroughly before every float (no exceptions)
19. I agree to lay in the tank with my head facing out towards the door and exit the tank immediately at the end of the float indicated by the music starting
20. I am not under the influence of drugs or alcohol
21. I agree to thoroughly read, understand and comply with all instructions given to me 

I declare this information to be true and accurate. I understand that this consent form is for today’s appointment and all future appointments at Infinity Float. I hereby confirm that I understand all statements above completely and take on all associated risks voluntarily. I hereby confirm that I understand this is a release of liability which could prevent me from filing suit or making claims for damages.
